Athletic Pre-Participation Physicals!

It's that time of year!  Time to check in with your physician and get your athletic pre-participation physical.  ALL student athletes, whether a returner or new to SWC, must get a physical with your personal physician or local clinic physician if you're from out of town.  

Print and complete the  2 forms attached here and take them to your appointment.  Reminder that the CCCAA mandates that you see an MD or DO, sorry, no nurse practitioners or physician assistants.  Don't lose your completed forms!  We'll be uploading them to your new electronic medical record soon.  

Call your doctor's office now to get scheduled before your first day of official practice!
